Mostly is from inactive glutes causing Anterior Pelvic tilt and overstriding. Not really to do with lean, you can lean forward and still have it.

Generally is from tight hip flexors and a lack of glute strength. Sometimes tight lower back. Often weak glute medius as well.

Another one:

You can see the far, non-supporting knee is “lower” due to the hip collapsing.
